Terms and Conditions of the Fizjo4Health Physiotherapy Clinic

§1 Legal Basis

The Fizjo4Health Physiotherapy Clinic operates based on:​
1) Applicable laws, including:
a)  The Act of April 15, 2011, on Medical Activity,
b)  The Act of September 25, 2015, on the Profession of Physiotherapist.
2) Registration in the register of entities performing medical activities, entry number: 000000267467, registered by the Mazovian Voivode on October 16, 2023.
3) These organizational regulations (hereinafter referred to as the "Regulations").​

§2 General Provisions

1. Fizjo4Health (hereinafter referred to as the "Clinic") is a company providing physiotherapy services.
2. The Clinic is located in Warsaw at 99 Korkowa Street.
3. These Regulations define the rules for providing physiotherapy services within the practice of the physiotherapist profession.
4. The Regulations are available at the Clinic and on the website https://fizjo4health.pl/.
5. Lack of knowledge of the Regulations does not exempt from compliance with its provisions.​

§3 Objectives and Tasks

1. The aim of the Clinic's staff is to provide physiotherapy and massage services by qualified personnel.  
2. The Clinic's tasks include ensuring high-quality physiotherapy services using known and proven therapeutic methods, adhering to professional ethics, promoting health prevention, and respecting patients' rights, considering their needs and safety.​

§4 Service Hours and Location

1. The Fizjo4Health Clinic is open from Monday to Friday, 8:00 AM to 7:00 PM (excluding public holidays).
2. Physiotherapy services are provided at the Clinic's premises.

§5 Appointment Booking and Cancellation Policy

1. Appointments can be booked by phone at +48 791 926 216, via the website https://fizjo4health.pl/ or through the Booksy app.
2. When booking, please provide your name, surname, and contact phone number.
3. Appointments can be rescheduled via the website, Booksy app, or by phone at least one day in advance, subject to availability.  
4. Cancellations can be made by phone, via the website, or directly in the Booksy app no later than one day before the scheduled appointment.
5. Cancellations made less than one day before the scheduled appointment will incur a fee of 50% of the treatment cost, added to the next visit's bill.
6. If a patient fails to attend two scheduled appointments, a 50% deposit will be required when booking the next session. This deposit is non-refundable if the patient does not attend the appointment.​

§6 Appointment Organization and Procedure

1. Patients are requested to arrive a few minutes before the scheduled appointment time.
2. Late arrivals will result in a shortened therapy session.
3. During the first visit, a medical interview and physical examination will be conducted to establish therapy goals and plans.
4. Before starting therapy, patients must provide written consent and personal data to create a patient card and maintain medical records.
5. The physiotherapist may refuse or discontinue therapy if:
a. The patient behaves inappropriately, violating the therapist's personal dignity,
b. The patient refuses to provide necessary personal data,
c. The therapy poses a health risk to the patient,  
d. The patient experiences a deterioration in well-being or skin changes during the session.
6. Patients will only undress to the extent required for therapy and will always be covered with a towel.
7. Fizjo4Health maintains medical records and ensures data protection.
8. Medical records can be accessed upon written request by:
a. The patient, their legal representative, or a person authorized by the patient,
b. Healthcare facilities and professionals if necessary for continuity of care,
c. Other entities authorized under applicable laws.​

§7 Patient Rights and Obligations

1. Patients have the right to:
a. Respect and courteous treatment,
b. Services provided by qualified personnel,
c. Accurate information about their health status and proposed therapies,
d. Full discretion,
e. Assistance and guidance on preparing for treatments,  
f. Consent to or refusal of treatment after receiving relevant information.
2. Patients are obliged to:
a. Inform staff about their health status, current and past conditions, and any contraindications,
b. Report any injuries, pain, or symptoms resulting from therapy,
c. Maintain personal hygiene,
d. Wear sports attire during therapy,
e. Follow the physiotherapist's recommendations,
f. Provide referrals and additional test results (e.g., ultrasound, X-ray, MRI) if available. Any injuries resulting from withholding health information or not following the physiotherapist's recommendations cannot be the basis for claims against Fizjo4Health.
